Diabetes Self-Management Education

December 26th, 2006

SARANAC LAKE - Diabetes Self-Management Education is often considered the cornerstone of care for people with diabetes who seek positive health related outcomes.
Taking control of diabetes prevents it from controlling you, however, taking control requires education. To help people with diabetes, Adirondack Medical Center will offer its Diabetes Self -Management Education Program starting January 17.
One of the key responsibilities of the diabetes care team is to provide accurate and up-to-date information about diabetes so that program participants can lead healthier and more fulfilling lives even though they have a chronic disease.
Meeting for six consecutive Wednesday evenings, the course is taught by a multi-disciplinary team consisting of four physicians: Dr. Claude Roland, vascular surgeon; Dr. David Johnson, internal medicine; Dr. Richard Erenstone, ophthalmologist; and Dr. David Thomashow, nephrologist; along with physical therapist Rick Preston, pharmacist Peter Henry, and dietitian/certified diabetes educator Sharon Sorgule. Registered nurses Ellen Winchell, Betsy Fuller, Peter Jones and Kathy Bonham round out the teaching staff.
The focus of the Diabetes Self-Management Program revolves around working with people with diabetes to design their self-management goals, provide education on how to take care of their disease, individualize medication, meal plans, exercise and activity, and learn how to recognize and treat complications of hyper- and hypoglycemia. New developments in this rapidly changing field will be highlighted.
This AMC program is accredited by the American Diabetes Association. A physician referral is required for insurance/Medicare reimbursement of the program fee. Scholarships are available through the AMC Foundation. Call the hospital at 897-2611 with questions or to register.
