AMC Offers Diabetes Self-Management Education Program

SARANAC LAKE - A team approach to diabetes care can effectively manage Type 1 and Type 2 diabetes and lower the risk for diabetes complications.

Team care emphasizes prevention of complications, coordination of care and shared decision making among people with diabetes and their health care providers.  This collaborative effort leads to a comprehensive lifetime management program.  

On January 18, Adirondack Medical Center, a member of Adirondack Health, will present its six-week Diabetes Self-Management Education Program.  Taught by an interdisciplinary staff of four physicians, a pharmacist, physical therapist, three registered nurses and a dietitian/certified diabetes educator, this comprehensive program is accredited by the American Diabetes Association.  It is designed for people recently diagnosed with diabetes, as well as those who are trying to fine tune their management skills. 

The program meets every Wednesday for six weeks from 6:30 to 8:30 p.m. in the Redfield Board Room at AMC in Saranac Lake.   A physician referral is needed for insurance, Medicaid and/or Medicare coverage.  Scholarships are available through the Adirondack Health Foundation.  Call (518) 897-2611 for information or to register.
